I thought they said that the NPR story showed that more of the dead people came from small towns. That's not the same thing. To see this, let's assume that there are a hundred people, 99 of which come from small towns, and one from a city. Now, choose one at random, with every one having an exactly equal chance. By definition, small townies are not more likely to be chosen, but the chances are 99 to 1 that a small townie will be chosen nevertheless.
